China/US researchers found semaglutide (Ozempic/Wegovy) protects osteoarthritis joints beyond mere weight loss. Acts directly on cartilage cells, boosting metabolism/energy production.
Osteoarthritis—most common arthritis—progressively wears protective cartilage. Obesity, diabetes, chronic inflammation accelerate, hitting bone, cartilage, synovial membrane.
Mouse experiments (obesity + osteoarthritis): semaglutide cut pain, slowed cartilage breakdown. Treated mice had fewer bone spurs, less joint membrane damage.
Weight-loss-only test: control group matched food intake/weight, yet lacked cartilage protection—direct joint health effect.
Cartilage analysis: ~8,300 protein expression changes signal deep biological reprogramming.
Drug Alters Cartilage Cell Metabolism

Molecular mechanism: semaglutide activates GLP-1R-AMPK-PFKFB3 axis—metabolic pathway regulating cell energy production.
Osteoarthritis chondrocytes rely on inefficient glycolysis (2 ATP/glucose). Post-treatment: shift to oxidative phosphorylation (up to 36 ATP/glucose)—better survival/tissue maintenance.
Small human trial (20 obese osteoarthritis patients, 50-75): one group got sodium hyaluronate (joint lubricant); other added semaglutide.
