Osteoporosis, a disease characterized by the progressive weakening of bones, affects millions worldwide. Its impact goes beyond physical health—fractures caused by this condition have significant economic and social consequences. Dominique Pioletti, from the Swiss Federal Institute of Technology Lausanne (EPFL), emphasizes that these consequences are often underestimated.

The numbers are striking: without proper prevention, approximately 40% of women and 20% of men over 50 will suffer severe osteoporosis-related fractures. Hip fractures are particularly alarming—mortality rates within the first year reach 20%, and more than half of affected individuals never regain their previous mobility or quality of life.

Currently, available treatments take months, sometimes over a year, to produce tangible results in bone regeneration. This leaves patients in prolonged vulnerability, where fracture risk remains high.

Faced with this challenge, an innovative alternative developed at EPFL could radically transform osteoporosis management.

Injectable Hydrogels: A Local Solution for a Global Problem

To accelerate bone strengthening, Dominique Pioletti’s team, through the startup Flowbone, has developed revolutionary hydrogel injections. This material, composed of hyaluronic acid and hydroxyapatite nanoparticles—a mineral naturally present in bones—significantly increases bone density in specific skeletal areas.

Unlike traditional treatments that act systemically, hydrogel injections offer a localized, much faster solution. In collaboration with Vincent Stadelmann from the Schulthess Clinic in Zurich, researchers demonstrated that combining this hydrogel with conventional medications could multiply local bone density four to five times.

Results in animal models are impressive. Rats treated exclusively with hydrogel injections showed a two- to threefold increase in local bone density. However, the most remarkable breakthrough occurred when hydrogel was combined with zoledronate, a common antiresorptive drug used for osteoporosis: within just two to four weeks, bone density increased by up to 4.8 times.

This accelerated effect is crucial in clinical practice, as it could protect patients during their highest-risk period—before systemic treatments begin to take effect over the long term.

Currently, Flowbone’s team is awaiting regulatory approvals to initiate human clinical trials. These studies will be critical in confirming the benefits observed in animal models and bringing this treatment into medical practice.

If successful, injectable hydrogels will not only accelerate bone recovery but also pave the way for more personalized therapies tailored to individual patients, minimizing risks while maximizing protection against devastating fractures.

With advances like this, the future of osteoporosis treatment looks safer, faster, and more effective—offering tangible hope for millions of people at risk worldwide.
